The Standardized Consent Form is a template developed by the European Commission to collect the consent of the patients to include their data in the CPMS system. This template needs to be approved by the corresponding ethical or legal committee of each hospital before using it. This template can be edited or modified to fulfil the centre requirements.
From ERN TransplantChild we have adapted this template and included the consent for the inclusion in the PETER registry. This way the professionals in our Network can provide the patients and their families with an unique document.
